Is Horizon Blue Cross Blue Shield Accepted Here?

Yes. Expert Rehabilitation Group is an in-network physical therapy provider with Horizon Blue Cross Blue Shield at both our Totowa and Nutley, New Jersey clinics.

Horizon is the largest health insurer in New Jersey, and Horizon members make up a significant share of the patients we treat. Whether you carry a Horizon plan through an employer, the state marketplace, or a State Health Benefits Program plan, you can be seen here at in-network rates.

What Being In Network Actually Means

Being in network means we have a contracted rate with the carrier, so you pay your plan’s in-network cost share rather than out-of-network rates. It does not mean every service is automatically covered or unlimited — coverage still depends on your individual plan, your deductible, and any visit limits or authorization rules it carries.

We treat orthopedic and musculoskeletal conditions exclusively: orthopedic physical therapy, post-surgical rehabilitation, spine care, and injury rehabilitation at our Totowa and Nutley clinics.

OMNIA and Tiered Horizon Plans

Horizon offers tiered network products — OMNIA Health Plans being the best known — where your out-of-pocket cost depends on whether a provider sits in Tier 1 or Tier 2. If you are on an OMNIA plan, this is the single most useful thing to confirm before you begin, because the difference in cost share can be meaningful.

Horizon also administers Horizon NJ Health (Medicaid) and Medicare Advantage products, which carry different rules from commercial plans. Tell us which type you have and we will check the right one.

Do You Need a Referral?

Under New Jersey’s Direct Access law, Dr. Sheldon Shterengarts and Dr. Roger Avecillas can evaluate and treat you without a prescription from another provider.

That is the law. Your insurance contract is separate — some plans still require a referral in order to pay, even though one is not required to treat. Confirming that distinction for your specific plan is part of what we check when we verify your benefits.

Horizon HMO and some tiered products may require a referral from your primary care physician for therapy to be covered. Horizon PPO and EPO plans generally do not. We confirm this for your plan before your first visit.

Before your first visit our bilingual staff contacts your plan directly and confirms your physical therapy benefits — whether a referral is required, how many visits are authorized, and what your copay or deductible responsibility will be. You will know your costs before treatment starts, not after.
